The Joint Commission

Condell Medical Center is committed to quality of care and patient safety.

The Joint Commission evaluates healthcare organizations on an ongoing basis to assess the compliance with nationally established Joint Commission standards. The standards address organizational quality and safety-of-care issues and the safety of the environment in which care is provided.

To our Patients and Visitors:

Any patient, family member, or visitor that has a concern about the care they or the patient has received, should feel free to contact us. You may access our online directory to locate the manager of the appropriate department.

We encourage you to let your care provider know if you have any concerns, questions or complaints about your care, treatment or safety. We want to resolve it as soon as possible. If you have voiced your concerns and feel it has not been resolved, please ask to speak to the Department Director or Manager. Or, you may call the Patient Representative Department at (847) 990-5443. If you have a concern about safety, you may also call the Confidential Patient Safety Hotline at (847) 990-5990.

Grievance Process:
If the Condell team has not been able to resolve your concerns to your satisfaction, you are welcome to take the next step by filing your grievance with either:
